New drug overcomes resistance in patients with rare sarcoma, study suggests

ScienceDaily (Nov. 21, 2012) ? A new targeted drug demonstrated its ability to control metastatic gastrointestinal stromal tumor, an uncommon and life-threatening form of sarcoma, after the disease had become resistant to all existing therapies, report investigators at Dana-Farber Cancer Institute who led the worldwide clinical trial.

The treatment of gastrointestinal stromal tumor, or GIST, even in its advanced metastatic stage, has been dramatically improved with two oral targeted drugs -- imatinib (Gleevec) and sunitinib (Sutent). To date, these have represented the only two FDA-approved treatments with the proven ability to control GIST. However, in more than 85 percent of patients, GIST becomes resistant to these drugs after seven years and the disease worsens with fatal results.

The new study, whose results are being published in The Lancet, demonstrated that the oral drug regorafenib, which inhibits several cancer-promoting kinase enzymes, was able to control GIST for nearly four months longer than placebo in patients for whom Gleevec and Sutent were no longer effective, a result that was highly significant statistically.

"When added to best supportive care, regorafenib significantly improves disease control, as measured by progression-free survival time in patients with GIST after progression which represents failure of all other therapies," said George Demetri, MD, of Dana-Farber, principal investigator of this clinical trial.

Demonstrating the aggressive nature of this resistant disease, the study found that tumors objectively grew in less than a month, on average, in GIST patients who were initially randomized to receive a placebo. The study's "cross-over" design made it possible to treat those patients whose tumors grew, and 85 percent of the patients initially on placebo were able to receive regorafenib, which then controlled the disease in these patients as well.

Because of the study's cross-over design, Demetri said, it was not expected to prove that the patients initially randomized to receive regorafenib survived longer -- the researchers would have had to withhold the drug from the placebo patients to demonstrate that difference. "But there is no question that people are living longer" with regorafenib treatment, he said, based on the results of this trial.

An application to have regorafenib approved for use in resistant GIST is under an accelerated review by the Food and Drug Administration, Demetri said.

GIST is a rare form of sarcoma that develops in the gastrointestinal tract, mainly in the stomach and small intestine. GIST is estimated to affect more than 5,000 people per year in the United States and about 8,000 in Europe.

Regorafenib is a novel rationally designed drug manufactured by Bayer HealthCare Pharmaceuticals that was FDA-approved in September 2012 to treat metastatic colon cancer after failure of standard chemotherapy. It blocks several cancer-promoting enzymes called kinases, which spur runaway growth in GIST and other cancers.

The phase 3 international trial involved 199 treatment-resistant GIST patients at 57 hospitals in 176 countries. Of the 199 patients, 133 received a regorafenib pill daily for three weeks followed by a one-week break, while 66 received a matching placebo. The patients were monitored for at least one year after the trial began.

As for other targeted therapies, the drug did not often shrink tumors but controlled the disease for an average of 4.8 months before it progressed, while patients in the placebo group experienced less than one month (0.9 month) before the disease worsened. There was a high rate of adverse effects including high blood pressure, fatigue, diarrhea, and redness, swelling, numbness and peeling of skin on the hands and feet. These side effects were managed by reducing or interrupting the regorafenib treatment, the report said.

A companion report in The Lancet said that the drug had a "modest" benefit in patients with metastatic colon cancer. A commentary by David Cunningham, MD, of the Royal Marsden Hospital in England, said, "In the relatively rare GIST, the case for routine use of this drug in patients following failure of existing treatments is strong."

Demetri added "We know that regorafenib can inhibit many of the mutated proteins and abnormal signals that cause this cancer, and the next step will be to investigate the molecular mechanisms by which this new treatment can control GIST after resistance appears to other 'targeted therapy' drugs for this aggressive malignancy."

The clinical trial was supported, in part, by Bayer HealthCare Pharmaceuticals, as well as the Ludwig Center at Dana-Farber/Harvard Cancer Center.

Sen. Bernie Sanders: Stand with Working Families

The Democrats won a major victory on Election Day.

Despite dozens of billionaires spending huge amounts of money to defeat President Barack Obama, he won a crushing victory in the Electoral College and received 3 million more votes than former Gov. Mitt Romney did nationally. Democrats won 25 of 33 seats contested in the Senate and, to everyone's surprise, expanded their majority there by two. They also gained seats in the House.

Now, with this victory behind them, the president and congressional Democrats must make it very clear that they will stand with the middle class and working families of our country. These are the people who, because of the Wall Street-caused recession, have seen a significant decline in their family income. These are the people who worry about whether they can afford health care and whether their kids will be able to attend college. The Democrats in the House and Senate must stand with these people -- not the millionaires and billionaires who are doing just fine.

Most important, in the coming weeks and months, the Democrats must demand that deficit reduction is done in a way that is fair -- and not on the backs of the elderly, the children, the sick and the poor. At a time when real unemployment remains close to 15 percent, we must also focus on creating the millions of jobs that our people need.

In America today, we have the most unequal distribution of wealth and income of any major country on Earth. Incredibly, the top 1 percent owns 42 percent of the nation's wealth while the bottom 60 percent owns just 2.3 percent. In the last study done on income distribution, we learned that 93 percent of all new income generated between 2009 and 2010 went to the top 1 percent while the bottom 99 percent split the remaining 7 percent. This extraordinary unfairness is not only morally reprehensible, it is bad economics. It will be very difficult to create the jobs that our people need when so many Americans have little or no money to spend.

Congress must pass legislation to create a major jobs program to put millions of people back to work rebuilding our crumbling infrastructure. Throughout our country, we need a massive effort to improve our roads, bridges, water and wastewater systems, airports, rail, broadband and cellphone service. Rebuilding our infrastructure makes us more productive and internationally competitive -- and creates millions of new jobs.

In terms of deficit reduction, let us not forget that in 2001, when Bill Clinton left office, this country had a $236 billion surplus. As a result of wars in Iraq and Afghanistan that were unpaid for, huge tax breaks for the rich, a Medicare prescription drug program put on the credit card and a significant decline in federal revenues because of the recession, we now have a $1 trillion deficit and a $16 trillion national debt.

Congress must address the deficit situation and the fiscal cliff, but we must do it in a way that is fair. At a time when the wealthiest people in this country are doing extremely well and their effective tax rates are low (think Romney), the people on top must pay their fair share of taxes to help us deal with the deficit. We must also end the outrageous loopholes that allow one out of four large profitable corporations to pay nothing in federal corporate income taxes. Further, it is absurd that current tax policy allows the wealthy and large corporations to avoid paying over $100 billion a year in federal taxes because they stash their money in tax havens in the Cayman Islands and elsewhere.

We must also take a hard look at wasteful spending in the Defense Department, where we now spend almost as much money as the rest of the world combined. Significant savings can be found at other federal agencies, too.

What we must not do, however, is move toward a balanced budget on the backs of the elderly, the children, the sick and the poor. Sadly, that is the approach that virtually all Republicans and some Democrats are advocating. As the founder of the Defending Social Security Caucus, I look forward to working with other members of Congress, the AFL-CIO, senior and disability groups and the vast majority of people in our country who want to prevent cuts in Social Security, Medicare, Medicaid, education and other programs vitally important to the working families of America.

In my view, if the Republicans continue to play an obstructionist role, the president should get out of the Oval Office and travel the country. If he does that, I believe that he will find that there is no state in the country, including those that are very red, where people believe that it makes sense to continue giving huge tax breaks to millionaires and billionaires while cutting Social Security, Medicare and Medicaid. I have a strong feeling that when large numbers of constituents all across this country start calling and emailing their senators and members of Congress about this issue, the American people will win this fight.

The good news is that we are already beginning to see some Republicans make thoughtful comments showing they understand that elections have consequences. Bill Kristol, the conservative commentator and Weekly Standard editor, said Sunday that the Republican Party should accept new ideas, including the much criticized suggestion by Democrats that taxes be allowed to go up on the wealthy. "It won't kill the country if we raise taxes a little bit on millionaires," he said on Fox News Sunday. "It really won't, I don't think. I don't really understand why Republicans don't take Obama's offer."

Kristol is right. At a time when the gap between the very rich and everybody else is growing wider, common sense and justice require the people who are doing extremely well financially to help us in a significant way to reduce deficits.

Two killed in suicide bomb attack near Kabul NATO base

KABUL (Reuters) - A suicide bomber killed two Afghan guards outside a NATO-run compound in the main diplomatic area of the Afghan capital, Kabul, on Wednesday, Interior Ministry officials said, an attack that was quickly claimed by the Taliban.

"The bomber killed himself in front of the compound. Two people were killed and two more were wounded, all Afghan guards," said Interior Ministry spokesman Najib Danesh.

A second man wearing an explosives-laden vest also approached the front of the compound but was shot dead by Afghan security forces, a statement from the Kabul police chief said. It said five civilians were also wounded.

The bomber struck near the sprawling base known as Camp Eggers, the headquarters of the NATO training mission and home to some 2,500 trainers from the multinational coalition.

Dozens of police swarmed to the area, where shattered glass from cars lay on the ground, and cordoned it off.

The Taliban took responsibility for the attack, which happened at 8.20 a.m. (0350 GMT) local time, Taliban spokesman Zabihullah Mujahid told Reuters by telephone from an undisclosed location.

Violence is intensifying across Afghanistan ahead of the planned withdrawal of most foreign troops by the end of 2014, sparking concerns over how the 350,000-strong Afghan security forces will be able to manage.

At least a dozen embassies, including the U.S. and British missions, and the headquarters of the NATO-led International Security Assistance Force (ISAF) are all near the area of the attack.

5 Tools To Keep Your Business Productive When The Office Is Shut ...

My friend Evan Stein of CMIT Solutions is one of the smartest technology consultants around ? here?s some advice from him.

5 Tools to Keep Your Business Productive:

1. Virtual Private Networking (VPN)
Virtual Private Networking allows you to connect your home PC to the company network as a secure node. VPN gives you access to all resources available to you on the office network such as server files, email, and applications that are server based.

2. Active remote desktop capabilities
CMIT?s Marathon Performance + and Ultra clients have remote access to their desktop available as part of their service plan. Remote desktop applications such as LogMeIn deliver your work desktop to your home PC, allowing you to run programs and view files on that machine as if you were sitting in front of your work PC. This simple and secure solution brings several other benefits to the table, including providing remote access for employees who work in the field or travel frequently, as long as there is a dedicated PC in the office for them to connect to. But what if your office loses power or Internet? You can?t remotely log onto a machine that doesn?t have electricity or access to the Internet.

3. Offsite backup
When your office loses power, maintaining productivity depends on having access to critical files. As an alternative to installing an onsite generator, having an offsite backup of your files in place will give you access to your data even when the lights go out. Such a system allows you to access an archived copy of your files that?s stored at an offsite location (normally a data center with its own generators and redundant Internet connections), which you should have anyway for purposes of data backups. Clients with CMIT?s Guardian backup have the benefit of offsite data backups and even remote virtualization.

4. Emergency access to email
Email is the lifeblood of many small businesses today. Having access to email without interruption is critical to business operations. An email archiving solution such as CMIT?s RADAR (Rapid Archiving, Discovery, and Recovery) has the additional capability to act as an emergency email system if your email server goes offline. Instead of using Outlook to send and receive email, during an event you would log into a web page and have the ability to view historical and new email, as well as compose and send new messages. An email archiving solution stores a copy of all emails and allows for fast searching, and as an added bonus, the email going to the archive doesn?t stop even if your email server loses power. Since many of us still depend on email, a searchable archive provides redundancy of this key communication tool in a cost-effective manner.

5. Implement an emergency texting service
In the case of a catastrophic natural disaster like a hurricane or flood, having an emergency texting service in place allows you to broadcast messages to your employees even if no one has access to email. Cellular networks tend to stay online even when other infrastructure experiences disruption, and mobile phones run on batteries, which makes this a solid solution for communicating during extreme weather events.

The key for all these tools, however, is having them in place and tested before disaster strikes. Additionally, these tools offer a myriad of other benefits to businesses in terms of cost savings, improvements to efficiency, data security, and system monitoring that continue to provide substantial ROI even when the snow has long since melted.
